This form is to be filled out prior to your meeting with Ann (and Leslie where appropriate). In our meeting we’ll discuss progress on this year’s goals, performance factors, and goals for 2002-03. Please let me know if you have any questions. Thanks!

1). Please write a brief narrative describing the following:

Goals and accomplishments for this year, including professional development. Please feel free to include career goals if you wish.
